Work-related stress according to the demand-control model and minor psychic disorders in nursing workers

This was a cross-sectional study that aimed to assess the association between work-related stress according to the Demand-Control Model, and the occurrence of Minor Psychic Disorder (MPD) in nursing workers. The participants were 335 professionals, out of which 245 were nursing technicians, aged predominantly between 20 and 40 years. Data were collected using the Job Stress Scale and the Self-Reporting Questionnaire-20. The analysis was performed using descriptive and analytical statistics. The prevalence of suspected MPD was 20.6%. Workers classified in the quadrants active job and high strain of the Demand-Control Model presented higher potential for developing MPD compared with those classified in the quadrant low strain. In conclusion, stress affects the mental health of workers and the aspects related to high psychological demands and high control still require further insight in order to understand their influence on the disease processes of nursing workers.

Capacidade para o trabalho na enfermagem: relação com demandas psicológicas e controle sobre o trabalho

OBJETIVO: avaliar a associacao entre demandas psicologicas, controle sobre o trabalho e a reducao da capacidade para o trabalho em trabalhadores de enfermagem. METODO: estudo transversal, envolvendo 498 trabalhadores de enfermagem de um hospital universitario do Estado do Rio Grande do Sul, Brasil. A coleta de dados foi realizada em 2009, utilizando-se as versoes brasileiras do Indice de Capacidade para o Trabalho e da Job Stress Scale e, para a analise dos dados, modelos de regressao logistica. RESULTADOS: evidenciou-se prevalencias de 43,3% e de 29,7% de reducao da capacidade laboral e de alta exigencia no trabalho (alta demanda psicologica e baixo controle), respectivamente. As chances de os trabalhadores em alta exigencia apresentarem reducao da capacidade para o trabalho foram maiores e significativas quando comparados aos classificados em baixa exigencia, mesmo apos ajustes por possiveis confundidores, exceto idade e sexo. CONCLUSAO: constatou-se elevada prevalencia de reducao da capacidade laboral. A evidencia alerta para a necessidade de investigacao e analise detalhada dos aspectos psicossociais, relacionados ao processo de saude/adoecimento dos trabalhadores de enfermagem.OBJECTIVE to evaluate the association between psychological demands, control over the work and the reduction of work ability of nursing professionals. METHOD this cross-sectional study involved 498 nursing professionals of a university hospital in the State of Rio Grande do Sul, Brazil. Data collection was carried out in 2009 using the Brazilian versions of the Work Ability Index and Job Stress Scale, with logistic regression models used for the data analysis. RESULTS the prevalence of 43.3% for reduced work ability and 29.7% for high-strain in the job (high psychological demand and low control) were observed. The chances for professionals presenting reduced work ability under high-strain were higher and significant when compared to those classified as being under low-strain, even after adjusting for potential confounders, except for age and gender. CONCLUSION a high prevalence of reduced work ability was observed. This evidence indicates the need for investigation and detailed analysis of the psychosocial aspects of the professionals with regard to the health/disease process of nursing professionals.

INTERNATIONAL COLUMN: Association of demographic, economic and clinical variables in daily activities and symptoms presented by patients in cancer treatment

Objective To investigate the association between demographic, economic and clinical variables, cancer symptoms, and daily life interference in patients receiving cancer treatment in Brazil. Methods In this cross-sectional study, 268 patients were assessed. A questionnaire was used to collect data on demographic, economic and clinical variables, and the M.D. Anderson Symptom Inventory was used to assess cancer symptoms. Data were analyzed using bivariate and multivariate descriptive statistics. Findings The following variables were associated with higher symptom scores: female sex (prevalence ratio [PR]=1.28; 95% confidence interval [95% CI] 1.06-1.53), illiteracy or ≤ 9 years of formal education (PR=1.40; 95% CI 1.08-1.82), clinical equipment or situations that requiring nursing care (PR=1.23; 95% CI 1.03-1.46), and family history of cancer (PR=1.23; 95% CI 1.04-1.45). Daily life interference was associated with female sex (PR=1.40; 95% CI 1.12-1.75), secondary tumour (PR=1.42; 95% CI 1.16-1.74) and radiotherapy (PR=1.24; 95% CI 1.01-1.51). Conclusion Management of cancer patients requires multidisciplinary knowledge, taking into consideration all the subjective dimensions of the patients. Knowing the profile of patients most strongly affected by symptoms will help them face the limitations and consequences of the disease and its treatment.

Qualidade da Atenção Primária à Saúde de crianças e adolescentes vivendo com HIV

Objetivo: avaliar a qualidade de atenção à saúde da criança e adolescente vivendo com HIV, entre os diferentes tipos de serviço de Atenção Primária à Saúde, de Santa Maria, Rio Grande do Sul. Método: estudo transversal, desenvolvido com 118 profissionais da Atenção Primária à Saúde. Utilizou-se o Instrumento de Avaliação da Atenção Primária, versão Profissionais. Para verificação das variáveis associadas ao alto escore, foi utilizada a Regressão de Poisson. Resultados: os profissionais da Estratégia Saúde da Família, quando comparados aos de Unidades Básicas de Saúde, obtiveram maior grau de orientação à atenção primária, tanto pelo escore geral quanto derivado, assim como nos atributos integralidade e orientação comunitária. Especialização em Atenção Primária à Saúde, outro emprego, vínculo estatutário mostraram-se associados à qualidade da atenção. Conclusão: destaca-se a Estratégia Saúde da Família com maior qualidade de atenção à saúde da criança e adolescente vivendo com HIV, entretanto, a cobertura ainda é baixa. Evidencia-se a necessidade de ampliação dessa cobertura, bem como investimento na qualificação profissional direcionada para Atenção Primária e na efetivação dos profissionais, por meio de concurso público, além de programa de aperfeiçoamento que reconheça a demanda do cuidado, nesses cenários, às crianças e adolescentes infectadas pelo HIV.Abstract Objective: to evaluate the quality of health care for children and adolescents living with HIV, among the different types of Primary Health Care services of Santa Maria, Rio Grande do Sul. Results: the professionals of the Family Health Strategy, when compared to those of the Primary Health Units, obtained a greater degree of orientation to primary care, both for the overall score and for the derived attributes score, as well as for the integrality and community orientation attributes. A specialization in Primary Health Care, other employment and a statutory work contract were associated with quality of care. Conclusion: the Family Health Strategy was shown to provide higher quality health care for children and adolescents living with HIV, however, the coverage is still low. The need was highlighted to expand this coverage and invest in vocational training directed toward Primary Care and making the professionals effective, through public selection procedure, as well as an improvement program that recognizes the care requirements, in these settings, of children and adolescents infected with HIV.
