Solid variant of keratocystic odontogenic tumor of the mandible: report of a case with a clear cell component and review of the literature

This is a case report on a solid variant of keratocystic odontogenic tumor arising in the mandible, which aggressively infiltrated into the cancelous spaces and involved the periosteal connective tissue of the mandible. The patient was a 57-year-old woman with an ill-defined radiolucent lesion having a moth-eaten pattern in the left molar region of the mandible. Computed tomography scans revealed that the tumor penetrated the buccal cortical plate of the mandible. Histologically, the lesion was characterized by multicystic spaces lined with a thin layer of keratinizing squamous epithelium, which contained basal cells with palisaded hyperchromatic nuclei. Lumina were filled with concentric layers of parakeratin. An additional feature was the appearance of a conspicuous clear cell component showing intraluminal papillary proliferation or forming small cord-like nests in the fibrous stroma. The patient underwent segmental mandibulectomy followed by reconstruction using a titanium plate. A 20-year follow-up revealed no recurrence of the tumor.

A Case of Bilateral Nasolabial Cysts

A case of bilateral nasolabial cysts in a 44-year-old female patient is reported. The patient was referred to our hospital with a chief complaint of swelling in the right alar base. Physical examination showed swelling with fluctuation in the oral vestibule of the bilateral maxillary canine areas. Yellow-colored fluid was aspired by fine needle aspiration cytology. CT scan showed two cystic lesions separately existing in the alar bases of the both sides. Under a clinical diagnosis of bilateral nasolabial cysts, the lesions were enucleated under general anesthesia. Histopathologically, the cystic cavities were lined by pseudostratified ciliated columnar epithelium with goblet cells.

Metal crown impacted in the tongue: a case report

We report a case of foreign body impaction in the tongue of a 63-year-old woman with schizophrenia. The patient was referred to our hospital by her dentist because of a swelling of the tongue. At the first visit to our hospital, physical examination revealed a submucosal elastic hard mass, measuring 23 × 20 mm, on the dorsal aspect of the tongue. There was a fistula in the mucosa over the mass. Oral antibiotics were given for a clinical diagnosis of a submucosal tumor with secondary infection, but the inflammation did not resolve completely. Computed tomography and radiography showed a metal crown impacted in the muscle layer of the tongue. The crown and surrounding soft tissue were removed with the patient under local anesthesia. Histopathologically, the resected tissue was inflamed granulation tissue with marked infiltration of neutrophils and proliferation of capillary vessels. No evidence of a neoplastic lesion was observed. A further medical interview made the patient confess to a history of tongue trauma caused by falling about 1 month before presentation to our hospital.
