O objetivo principal deste artigo e apresentar uma orientacao teorica e pratica aos preceptores da atencao primaria a saude vinculados a Universidade Federal de Pernambuco. Pressupoe a atencao primaria a saude como um cenario de pratica para estudantes de diferentes graduacoes e pos-graduacoes com caracteristicas proprias. Aborda aspectos importantes do processo ensino-aprendizagem no ambiente comunitario, articulando os diferentes atores deste, entre os quais o conceito de conhecimento na atencao primaria; a relacao preceptor-estudante; a correlacao teoria e pratica; a avaliacao processual; o ensino e a pesquisa no trabalho; a insercao politico-social da aprendizagem; a interdisciplinaridade; o trabalho em equipe e a interinstitucionalidade na integracao ensino-servico. Por fim, propoe a reintegracao do espaco da educacao, da pesquisa e do trabalho.

These are a set of guidelines built by the Brazilian Association of Medical Education (ABEM) and the Brazilian Society of Family and Community Medicine (SBMFC) with the aim of supporting medical schools in a practical and objective manner, when elaborating pedagogical-political projects on Primary Health Care (PHC). The advent of the Brazilian National Curricular Guidelines for Medical Education, which are approved by the Ministry of Education in 2001 have since improved the teaching of undergraduate medical students on PHC, but there are still wide variations in implementation and quality of it in medical curricula. These guidelines by ABEM/SBMFC partnership can exert considerable influence on medical curricula by establishing minimum requirements and core competencies for PHC in Brazil.

The occurrence of leprosy has decreased in the world but the perspective of its elimination has been questioned. A proposed control measure is the use of post-exposure chemoprophylaxis (PEP) among contacts, but there are still questions about its operational aspects. In this text we discuss the evidence available in literature, explain some concepts in epidemiology commonly used in the research on this topic, analyze the appropriateness of implementing PEP in the context of Brazil, and answer a set of key questions. We argue some points: (1) the number of contacts that need to receive PEP in order to prevent one additional case of disease is not easy to be generalized from the studies; (2) areas covered by the family health program are the priority settings where PEP could be implemented; (3) there is no need for a second dose; (4) risk for drug resistance seems to be very small; (5) the usefulness of a serological test to identify a higher risk group of individuals among contacts is questionable. Given that, we recommend that, if it is decided to start PEP in Brazil, it should start on a small scale and, as new evidence can be generated in terms of feasibility, sustainability and impact, it could move up a scale, or not, for a wider intervention.

INTRODUCTION: Over the last two years, medical courses in Brazil have begun implementing a compulsory internship in five basic areas of medicine, referred to as a clinical clerkship. Municipal and state health services are currently venues for these internships. This study aims to investigate the reality of such a clinical clerkship at the Medical School of the Federal University of Pernambuco from the point of view of its mentors. METHODOLOGY: This is a descriptive, qualitative, cross-sectional study in which we interviewed mentors from health services involved in the clinical clerkship. RESULTS: The themes most relevant to the study were: the mentors expectations of the medical training during the rotation; the mentorship teaching models; education management; and teaching-service integration. CONCLUSIONS: Specific teaching-service integration initiatives may qualify placements in the health network, and these initiatives should address the topics related to the categories defined in this study. Other studies might address the perspectives of the student, teacher and manager, in an important step towards complementing the information obtained in this study.

According to Brazilian health policy, the countrys healthcare guidelines include what are defined as standardized actions, established according to specific program protocols, and unique actions such as humanization. In primary healthcare (PHC), the uptake of the different actions by health system managers and health professionals has occurred gradually, through distinct interventions. The current study aimed to analyze the ability of a family health team (FHT) in a real context to achieve the proposed targets for programmatic actions, combined with the development of unique actions. Based on Brazilian and international parameters for medical procedures under PHC, the study included healthcare capacity projections using models for programming results and calculating rate of care pressure (Stockmeier equation) for a given FHT. The projections were then presented to the FHT in a focus group format, in which the contributions were analyzed qualitatively. Thus, with the research results, the current study is useful for backing a revision of PHC goals and parameters.
