Electromyographic indices, orofacial myofunctional status and temporomandibular disorders severity: A correlation study.

This study examined whether there is an association between surface electromyography (EMG) of masticatory muscles, orofacial myofunction status and temporomandibular disorder (TMD) severity scores. Forty-two women with TMD (mean 30 years, SD 8) and 18 healthy women (mean 26 years, SD 6) were examined. According to the Research Diagnostic Criteria for TMD (RDC/TMD), all patients had myogenous disorders plus disk displacements with reduction. Surface EMG of masseter and temporal muscles was performed during maximum teeth clenching either on cotton rolls or in intercuspal position. Standardized EMG indices were obtained. Validated protocols were used to determine the perception severity of TMD and to assess orofacial myofunctional status. TMD patients showed more asymmetry between right and left muscle pairs, and more unbalanced contractile activities of contralateral masseter and temporal muscles (p<0.05, t-test), worse orofacial myofunction status and higher TMD severity scores (p<0.05, Mann-Whitney test) than healthy subjects. Spearman coefficient revealed significant correlations between EMG indices, orofacial myofunctional status and TMD severity (p<0.05). In conclusion, these methods will provide useful information for TMD diagnosis and future therapeutic planning.

Correlação entre métodos de aleitamento, hábitos de sucção e comportamentos orofaciais

BACKGROUND The development of oral motor control depends partially on motor and sensory experiences. AIM To analyze the relationship between the duration of breastfeeding, artificial feeding and sucking habits, and of these parameters with the orofacial motor performance. METHOD Participants of this study were one hundred and seventy-six children aged 6 to 12 years. All subjects were submitted to an orofacial myofunctional clinical examination, using a protocol with scores, and parents/care takers were interviewed in respect to the feeding and sucking habits of their children. Correlations were calculated using the Spearman Test. RESULTS In the studied sample, the mean duration of breastfeeding was of 10.30 months (ranging from zero to 60 months), of artificial feeding was of 44.12 months (zero to 122 months) and of sucking habits was of 39.32 months (0 to 144 months). There was a negative correlation of breastfeeding duration with artificial feeding duration and sucking habits duration (p < 0001). The duration of the artificial feeding was positively correlated to the duration of sucking habits (p < 0001). The duration of breastfeeding was positively correlated with the mobility of the tongue and jaw (p = 0.05). There was a negative correlation of the duration of artificial feeding and the duration of sucking habits with the performance in mastication and swallowing, respectively, as well as with the duration of both types of sucking with the performance in the diadochokinesia test (p = 0.05). CONCLUSION The duration of natural breastfeeding presented a positive effect over the mobility of the orofacial structures. Deleterious effects of the prolonged duration of artificial feeding and sucking habits in the oral motor control were confirmed.

Validity of the 'protocol of oro-facial myofunctional evaluation with scores' for young and adult subjects.

UNLABELLED The aims of this study were to analyse the validity, sensitivity and specificity of the protocol of oro-facial myofunctional evaluation with scores (OMES) for oro-facial myofunctional disorder (OMD) diagnosis in young and adult subjects. Eighty subjects were examined. The OMES was validated against the Nordic orofacial test-screening (NOT-S) protocol (criterion validity) (Spearman correlation test). The construct validity was tested by analysis of the ability of the OMES (i) to differentiate healthy subjects (n = 22) from temporomandibular disorder (TMD) patients (n = 22), which frequently have OMD (Mann-Whitney test) and (ii) to measure the changes that occurred in a subgroup with TMD between the period before and after oro-facial myofunctional therapy (T group, n = 15) (Wilcoxon test). Two speech therapists trained with the OMES participated as examiners (E). There was a statistically significant correlation between the OMES and NOT-S protocols, which was negative because the two scales are inverse (r = -0·86, P < 0·01). There was a significant difference between the healthy and TMD subjects regarding the oro-facial myofunctional status (OMES total score, P = 0·003). After therapy, the T group showed improvement in the oro-facial myofunctional status (OMES total score, P = 0·001). Inter- and intra-examiner agreement was moderate, and the reliability coefficients ranged from good to excellent. The OMES protocol presented mean sensitivity and specificity = 0·80, positive predictive value = 0·76 and negative predictive value = 0·84. CONCLUSION The OMES protocol is valid and reliable for clinical evaluation of young and adult subjects, among them patients with TMD.

Limites de movimentos mandibulares em crianças

TEMA: a determinacao dos limites de movimentos mandibulares e um importante procedimento na avaliacao do estado funcional do sistema estomatognatico, porem poucos sao os estudos que focalizam os parâmetros de normalidade ou desvios em criancas. Objetivos: definir as medias dos limites de movimentos mandibulares em criancas brasileiras de 6 a 12 anos de idade; verificar diferencas entre os generos, em cada faixa etaria, e entre as faixas etaria de 6 a 8 anos, 8:1 a 10 anos e 10:1 a 12 anos de idade. METODO: participaram 240 criancas, escolares do interior do Estado de Sao Paulo. Com o auxilio de um paquimetro digital foram mensuradas a maxima abertura mandibular, a protrusao, a excursao lateral direita e esquerda e o desvio da linha media, quando presente. O teste T Student, a Analise de variância e o pos-teste Tukey foram considerados significantes para p < 0,05. RESULTADOS: as medias das medidas da amostra foram: maxima abertura mandibular 44,51 mm, excursao lateral direita 7,71mm, excursao lateral esquerda 7,92 mm e a protrusao 7,45 mm. Nao houve diferenca estatistica entre os generos. Houve aumento gradual nos limites dos movimentos mandibulares com o aumento da faixa etaria, com diferencas significantes principalmente entre as faixas etarias de 6 - 8 anos e 10:1-12 anos. CONCLUSAO: durante a infância os limites de movimentos mandibulares aumentam e a idade deve ser considerada na analise desses dados para maior precisao no diagnostico.

Orofacial Myofunctional Evaluation Protocol for older people: validity, psychometric properties, and association with oral health and age

PURPOSE To develop a comprehensive assessment protocol for identifying, classifying and grading changes in stomatognathic system components and functions of older people, to determine its psychometric properties and verify its association with oral health and age. METHODS The content validity of the Orofacial Myofunctional Evaluation with Scores for Elders protocol (OMES-Elders) was established based on the literature. The protocol contains three domains: appearance/posture, mobility, and functions of the stomatognathic system. Eighty-two healthy elder volunteers (mean age 69±7.24 years) were evaluated using the OMES-Elders. A test-screening for orofacial disorders (reference) was used to analyze the concurrent validity (correlation test), sensitivity, specificity and accuracy (Receiver Operating Characteristic Curve: ROC curve) of the OMES-Elders. The association of the OMES-Elders scores with the Oral Health Index (OHX) and age in the sample was tested. RESULTS There was a significant correlation between the OMES-Elders and the reference test (p < 0.001). Reliability coefficients ranged from good (0.89) to excellent (0.99). The OMES-Elders protocol had a sensitivity of 82.9%, specificity of 83.3% and accuracy of 0.83. The scores of the protocol were significantly lower in individuals with worse oral health (OHX ≤ 61%), although individuals with adequate oral health (OHX ≥ 90%) also had myofunctional impairments. The predictors OHX and age explained, respectively, 33% and 30% of the variance in the OMES-Elders total score. CONCLUSION As the first specific orofacial myofunctional evaluation of older people, the OMES-Elders protocol proved to be valid, reliable and its total score was associated with oral health and age.

A Tendência Antissocial em Crianças: uma Revisão Integrativa da Literatura

The present work aims to carry out a review of literature that involves the topics: antisocial tendency, family and childhood. To do this, searches were conducted in the databases of the Virtual Health Library, Capes Portal and ERIC. The keywords used were: antisocial tendency and family, being searches in Portuguese, English and Spanish. They were selected for reading in their entirety and 14 analysis articles, being five in Portuguese, six in English and three in Spanish. Generally speaking, the articles seek to investigate the origin of the antisocial tendency; to check how the relationship of the child with his/her family is; to analyze gender differences in relation to the disorder and to discuss intervention strategies. The analysis of the works points to the importance of a greater family participation in the childs development, so that his/her needs are met. In this sense, interventions must be carried out with the whole family. The need of constant research on the subject is pointed out, in order to better understand their origins and forms of intervention.
