Primeiro episódio da esquizofrenia e assistência de enfermagem

Schizophrenia is one of the main health problems in current days, requiring considerable investment from the health system. Intervening in the first episode offers a unique opportunity in the treatment of schizophrenia and influences the course of the illness. This article consists of a critical literature review aimed at examining knowledge on first episode schizophrenia and discussing the contribution of nursing care. A research was carried out in bibliographical databases. The data collected made possible the organization of information on the general concept of schizophrenia, its first episode, types of intervention and nursing performance. We found out that in Brazil there are few studies related to first episode schizophrenia in Nursing, few available specialized services, and few social resources. This situation reveals the need for more studies on first episode schizophrenia.A esquizofrenia e um dos principais problemas de saude da atualidade, exigindo consideravel investimento do sistema de saude. A intervencao no primeiro episodio do transtorno oferece uma oportunidade unica no tratamento da esquizofrenia, influenciando no curso da doenca. O presente artigo consiste em uma revisao critica de literatura cujos objetivos sao examinar o conhecimento sobre o primeiro surto de esquizofrenia e discutir a contribuicao da enfermagem na assistencia. Foi utilizada pesquisa bibliografica em indice informatizado de referencias. Os dados obtidos permitiram organizar informacoes sobre o conceito geral de esquizofrenia, seu primeiro surto, tipos de intervencoes e a atuacao de enfermagem. Observamos que existe pouca literatura brasileira relacionada ao primeiro surto esquizofrenico, na area da enfermagem, poucos servicos especializados e disponiveis e poucos recursos sociais. Tal condicao mostra a necessidade de estudos relacionados ao primeiro surto.

Pesquisas com famílias de portadores de transtorno mental

La finalidad de este trabajo es presentar una reflexion critica de los conocimientos producidos acerca de los temas: el impacto del trastorno mental en la familia, el impacto en el ambiente familiar en la evolucion de los trastornos y la experiencia de la familia con la enfermedad mental. Las medidas de sobrecarga y de emocion expresas son utiles para orientar politicas publicas y la practica profesional, pero no nos da un cuadro multidimensional de la experiencia del cuidado. Investigaciones sobre la experiencia familiar contribuyen para la comprension del sufrimiento presente en esta experiencia y de las formas de enfrentamiento adoptadas por las familias, contribuyendo tambien para que percibamos las diferencias de los contextos culturales en el enfrentamiento de las familias.The purpose of this paper is to present a critical reflection on the knowledge produced on the topics: the impact of the mental illness in the family, the impact of the family environment in the evolution of the eating and the experience family with the mental illness. The measures of overload and express emotion are useful to guide public politics and the practice professional, however in they do not give a multidimensional picture to them of the experience of the care. Research on the familiar experience contribute to the understanding of the present suffering in this experience and of the forms of confrontation adopted by the families, also contributing to perceive the differences of the cultural contexts in the confrontation of the families.

Research with families of people with mental disorder

Ajustamento familiar após o surgimento da esquizofrenia

It is considered that, when there is the first episode of schizophrenia, family life is disrupted and their life trajectory can be modified. In view of this, the aim of this study was to know the trajectory of families in the first five years of living with mental illness, identifying the adjustment process. The research was based in Symbolic Interactionism. The individual interviews were realized with 23 relatives of 21 patients with schizophrenia. The analysis aimed at determining a timeline, which allowed us to describe the process of family adjustment through three times: Perceiving the Change, Diagnosis and Begin of Treatment and Following in Front. All phases presented in this project are important and can be more studied.Se considera que, cuando ocurre el primer episodio de la esquizofrenia, la vida familiar se rompe y su trayectoria de vida puede ser modificada. En vista de eso, el objetivo de este estudio fue conocer la trayectoria de las familias en los primeros cinco anos de vivir con una enfermedad mental, identificando el proceso de adaptacion. El referencial teorico metodologico utilizado se basa en el Interaccionismo Simbolico. Se realizaron entrevistas individuales con 23 familiares de 21 portadores de esquizofrenia. El analisis tuvo como objetivo determinar una linea de tiempo, lo que nos permitio describir el proceso de adaptacion de la familia a traves de tres momentos: Percibiendo el Cambio, Diagnostico e Inicio del Tratamiento y Siguiendo Adelante. Concluye-se que todas las fases presentadas en este proyecto son ricas y pueden seguir siendo investigadas.

Mental health in the Family Health Strategy as perceived by health professionals.

Objective: to analyze the management of mental health needs in primary care as perceived by Family Health Strategy professionals. Method: this was a qualitative descriptive exploratory study developed within the coverage area of five family health teams. The data were collected using observation, group interviews, individual semi-structured interviews, and focus groups. Content analysis was conducted using text analysis software and interpretation was based on the corresponding analytical structures. Results: numerous and challenging mental health demands occur in this setting, for which the teams identified care resources; however, they also indicated difficulties, especially related to the operationalization and integration of such resources. Conclusion: there is a need for a care network sensitive to mental health demands that are better coordinated and more effectively managed. Objetivo: analisar o manejo das necessidades de saúde mental na atenção primária à saúde de acordo com a percepção dos profissionais da Estratégia Saúde da Família. Método: estudo qualitativo, descritivo exploratório, desenvolvido no território de abrangência de cinco equipes de saúde da família. Os participantes foram cinco enfermeiras, cinco coordenadores e 17 agentes comunitários de saúde. Os dados foram coletados utilizando observação, entrevistas grupais, entrevistas individuais semiestruturadas e grupos focais. Fez-se a análise de conteúdo com o auxílio de um Software de análise textual, e a interpretação baseou-se nas estruturas analíticas correspondentes. Resultados: inúmeras e desafiadoras demandas de saúde mental têm sido acolhidas nesse setting, para as quais as equipes identificaram recursos de atendimento; no entanto, apontaram dificuldades, sobretudo relacionadas à operacionalização e integração destes recursos. Conclusão: destaca-se a necessidade de uma rede de cuidados sensível a tais demandas, mais articulada e gerida de modo eficaz.
