The cultural adaptation and validation of the "Burn Specific Health Scale-Revised" (BSHS-R): version for Brazilian burn victims.

BACKGROUND The Burns Specific Health Scale-Revised (BSHS-R) is of easy application, can be self-administered, and it is considered a good scale to evaluate various important life aspects of burn victims. OBJECTIVES To translate and culturally adapt the BSHS-R into the Brazilian-Portuguese language and to evaluate the internal consistency and convergent validity of the translated BSHS-R. METHODS The cultural adaptation of the BSHS-R included translation and back-translation, discussions with professionals and patients to ensure conceptual equivalence, semantic evaluation, and pre-test of the instrument. The Final Brazilian-Portuguese Version (FBPV) of the BSHS-R was tested on a group of 115 burn patients for internal consistency and validity of construct (using the Rosenberg Self-Esteem Scale (RSES) and the Beck Depression Inventory (BDI)). RESULTS All values of Cronbachs alpha were greater than .8, demonstrating that the internal consistency of the FBPV was very high. Self-esteem was highly correlated with affect and body image (r=.59, p<.001), and with interpersonal relationships (r=.51, p<.001). Correlations between the domains of the FBPV and the BDI were all negative but larger in magnitude than the correlations with RSES. Depression was highly correlated with affect and body image (r=-.77, p<.001), and with interpersonal relationships (r=-.67, p<.001). CONCLUSIONS The results showed that the adapted version of the BSHS-R into Brazilian-Portuguese fulfills the validity and reliability criteria required from an instrument of health status assessment for burn patients.

Prevenção de queimaduras: percepção de pacientes e de seus familiares

Os objetivos deste estudo foram investigar os meios de prevencao de queimaduras identificados por pacientes vitimas de queimaduras e por seus familiares. Os dados foram coletados na Unidade de Queimados do Hospital das Clinicas da Faculdade de Medicina de Ribeirao Preto, mediante a realizacao de entrevistas com pacientes e seus familiares. Os dados foram categorizados por quatro juizes. Para verificar a existencia de concordância entre os juizes, quanto a categorizacao, foi utilizada a medida nao parametrica de correlacao - Coeficiente de Contingencia. Foram entrevistados: 24 familiares de pacientes que sofreram queimaduras e 33 pacientes. Dos 33 pacientes entrevistados, 18 sofreram acidentes em ambiente domestico. Dos 57 entrevistados, 11 afirmaram que nao poderiam ter contribuido para evitar o acidente. Vinte pacientes e 12 familiares identificaram situacoes de risco no ambiente domestico ou no trabalho, e 13 pacientes e 12 familiares nao identificaram qualquer tipo de situacao de risco. Os familiares e pacientes apontaram como medidas preventivas de acidentes por queimaduras: estar atento as atividades realizadas e ter cuidado com o manuseio de produtos inflamaveis.

Curativo do paciente queimado: uma revisão de literatura

O objetivo deste estudo foi descrever, mediante revisao de literatura, os agentes topicos e tipos de cobertura que tem sido recomendados no tratamento de queimaduras e discutir as implicacoes do uso desses produtos. O estudo foi desenvolvido mediante levantamento bibliografico dos ultimos 12 anos, nas bases de dados Lilacs, Medline e Dedalus. As publicacoes encontradas foram organizadas como de pesquisa e de revisao e, posteriormente, categorizadas de acordo com a tematica: agentes topicos e substitutos temporarios de pele. Foram discutidos os possiveis efeitos colaterais, indicacoes e recomendacoes quanto a manipulacao.This study aimed at describing, by means of a literature review, the topical agents and bandage types which have been recommended in the treatment of burns and their implications in nursing care. This study was conducted by collected bibliographic data concerning the last 12 years from Lilacs, Medline and Dedalus databases. A experiência da queimadura na perspectiva do paciente

This study was aimed at identifying situations experienced by burned patients which they perceived as significant during hospitalization, as well as identifying the feelings and consequences that resulted from the reported situations. The Critical Incident Technique was used for data collection, based on descriptions of facts which patients had previously observed and experienced as referred to by their memories. Thirty patients were submitted to semi-structured interviews. The situations identified as significant were categorized as follows: feeling pain and having to subject themselves to procedures; having a new experience, receiving family and team support or not; and knowing the situation of other burned persons. The feelings and behaviors that were mentioned were anxiety and fear, and suffering and comfort when seeing others whose conditions are worse. From the perspective of patients, the consequences of going through this experience were changes in values, lifestyle, social role and health.This study was aimed at identifying situations experienced by burned patients which they perceived as significant during hospitalization, as well as identifying the feelings and consequences that resulted from the reported situations. Suicide attempted by burning in Brazil

From 1984 through 1995, a total of 82 patients were admitted to the Burn Center of the University Hospital of Ribeirão Preto, University of São Paulo with a history of attempted suicide. Fifty-eight patients were female and 24 were male. The mean burned surface area (BSA) was 52.4 per cent and the mortality rate was 44 per cent. Methyl alcohol was used by 87.5 per cent of the patients to ignite the fire.

Analysis of cost of dressings in the care of burn patients

This study aimed at analyzing the costs of material used in the occlusive dressings of burn patients. The subjects of the study were patients over the age of 18 years who were admitted to the Burns Unit of the Ribeirão Preto Medical School Clinical Hospital, University of São Paulo-Brazil, from 15 January to 11 September 1998. Data were collected concerning the costs and duration of bathing and dressing of these patients. We concluded that there was a positive relation between the real costs of the materials used and the percentage of the body surface burned. In addition, there was a positive correlation between the number of staff involved in care and the size of the burns. There was no significant difference between the cost of materials used in dressings for burns of different depths.

Planejamento da assistência a pacientes vítimas de queimaduras: relação entre os problemas registrados e cuidados prescritos

O objetivo deste estudo foi identificar os diagnosticos de enfermagem de pacientes adultos, admitidos em uma Unidade de Queimados, com base nos registros de enfermagem realizados nas primeiras 72 horas apos a queimadura, e analisar se haveria relacao entre os diagnosticos identificados e os cuidados prescritos pelos enfermeiros. Avaliaram-se os registros de enfermagem em 42 prontuarios de pacientes e estabelecidos os diagnosticos de enfermagem. Para cada diagnostico, relacionaram-se as intervencoes prescritas pelos enfermeiros. Foi possivel identificar 13 diferentes diagnosticos de enfermagem. Foram propostas prescricoes para os diagnosticos identificados com base na literatura.This study was aimed at identifying nursing diagnoses of adult patients hospitalized at a Burn Trauma Unit based on nursing records made in the first 72 hours after the burn and at analyzing if there is a relation between the identified diagnoses and the care prescribed by the nurses. The nursing records of 42 patients were assessed and the nursing diagnoses were established. For each diagnosis the interventions prescribed by the nurses were listed. Thirteen different nursing diagnoses could be identified. We observed cases of care prescribed without the record of an associated problem and diagnoses for which there were no prescriptions. There were prescriptions proposed for the diagnoses that were identified based on the literature.

Diagnósticos de enfermagem presentes em familiares de pacientes vítimas de queimaduras

This study is aimed at investigating the nursing diagnoses of family members of burn victims. Seventeen family members were evaluated through observation and interviews. Nursing diagnoses were established on the basis of NANDA Taxonomy II. Eighteen different nursing diagnoses were identified. The diagnoses: anxiety, fear and anticipatory grieving were characterized through the manifestation of despair in the face of the possibility of losing a loved one. Some diagnoses possibly had already been present before the occurrence of the accident, such as, for example: inefficient role performance, impaired home maintenance, altered parenting.O objetivo deste estudo foi investigar os diagnosticos de enfermagem de familiares de pacientes vitimas de queimaduras. Dezessete membros da familia de pacientes vitimas de queimaduras foram avaliados por meio de observacao e entrevista. Os diagnosticos de enfermagem foram estabelecidos com base na Taxionomia II da NANDA. Foram identificados 18 diferentes diagnosticos de enfermagem. Os diagnosticos: ansiedade, medo e sentimentos de pesar antecipado se caracterizaram pela manifestacao de desespero ante a possibilidade da perda de um ente querido. Alguns diagnosticos, possivelmente, ja estavam presentes antes do acidente ocorrer, como, por exemplo: desempenho de papeis ineficazes, manutencao do lar prejudicada, paternidade e maternidade prejudicadas.
