Risk perception in family health work: study with workers in Southern Brazil

This analytical and qualitative study aimed to identify how workers in the Family Health Strategy perceive the risks they are exposed to at work. Thematic analysis and the reference framework of the work process were used to examine the contents of interviews with 48 subjects (community health agents, nurses, nursing auxiliaries and physicians). The workers noticed the following risks: physical and moral violence, typical work accident, emotional exhaustion, lack of problem-solving ability and occupational disease. The results predominantly show the connection with the object / subject of the work and the social environmental characteristics of the related communities. Processo de trabalho da Estratégia Saúde da Família: a concepção de gestão que permeia o agir em saúde

Este estudo, de carater explicativo e transversal, e natureza predominantemente qualitativa, tem o objetivo de analisar o processo de trabalho dos gestores municipais da Estrategia Saude da Familia (ESF), a fim de apreender a concepcao de gestao que permeia seu agir em saude. Os dados foram coletados atraves de entrevista semiestruturada gravada, junto aos gestores da ESF pertencentes a 12 municipios da Terceira Coordenadoria Regional de Saude do Rio Grande do Sul (3aCRS/RS), totalizando 15 sujeitos. A analise tematica congregou uma abordagem dialetica, seguindo os passos de pre-analise, exploracao e tratamento do material e interpretacao dos resultados. Os resultados foram organizados em tres subtemas elucidativos das acoes desenvolvidas pelos gestores investigados: 1) Acoes de organizacao/funcionamento da Saude da Familia; 2) Acoes de formacao/educacao permanente ou continuada; 3) Acoes de monitoramento e avaliacao. Apreendem-se do processo de trabalho duas concepcoes de gestao, sendo a primeira relacionada a gestao classica/tradicional e a segunda relativa aos preceitos da cogestao. A guisa de conclusao, depreende-se que o processo de trabalho dos gestores da ESF na 3aCRS/RS apresenta fortalezas e fragilidades, relacionadas a concepcao de gestao que estrutura seu trabalho. Assim, sugere-se a revisao de algumas praticas, de modo que a cogestao seja a concepcao preferencial utilizada para a administracao da Saude da Familia.

Infant mortality and prenatal care: contributions of the clinic in the light of Canguilhem and Foucault

Neste estudo de revisao objetivou-se verificar como pesquisas realizadas no Brasil relacionam a mortalidade infantil e o pre-natal e apresentar contribuicoes da clinica a luz de Canguilhem e Foucault, para a qualificacao da assistencia. Realizou-se revisao integrativa de literatura a partir de buscas nas bases de dados SciELO, LILACS, MEDLINE e BDENF, no periodo de 2000 a 2009. A relacao entre a mortalidade infantil e o pre-natal refere-se ao numero insuficiente de consultas ou a qualidade do atendimento prestado. Mesmo quando o numero e a rotina de consultas no pre-natal foram adequados, os obitos evitaveis estiveram presentes. Para a qualificacao da assistencia oferecida, sugere-se que sejam considerados o conhecimento clinico e os demais elementos componentes do processo de viver humano, no intuito de que o olhar clinico seja ampliado e articulado as tecnologias disponiveis no sistema de saude e, em conjunto, consigam contribuir para a reducao da mortalidade infantil no Brasil.This review study aimed to verify how studies conducted in Brazil have related infant mortality to prenatal care and to present contributions of the clinic in the light of Canguilhem and Foucault for qualification of the care. An integrative literature review was conducted from searches in the databases SciELO, LILACS, MEDLINE and BDENF for the period 2000 to 2009. The relationship between infant mortality and prenatal care is related to the insufficient number of consultations or to the quality of the care provided. Even when the number of and routine consultations in the prenatal care were adequate, avoidable deaths were present. For the qualification of prenatal care, it is suggested that the clinical knowledge and other elements that comprise the process of human living are considered, in order that the clinical view is enlarged and articulated to the technologies available in the health system and, together, they are able to contribute to the reduction of infant mortality in Brazil.

The non-implementation of the nursing process: reflection based on Deleuze's and Guattari's concepts

El proceso de enfermeria se constituye una fortaleza en la ciencia de Enfermeria, pues contribuye para su consolidacion, ya que explicita la sistematizacion del su pensar y hacer, dando visibilidad al trabajo que es realizado. Todavia, hay muchas dificultades en su implementacion, reportadas en estudios nacionales e internacionales. Esas dificultades han motivado la construccion de este texto, que tuvo por objetivo reflexionar acerca del trabajo de enfermeria, a partir de una reflexion teorica apoyada en aspectos de la subjetividad, de acuerdo con Deleuze y Guattari, llevando en cuenta que ellos pueden ser inconscientemente expresados por las enfermeras en su trabajo diario, afectando la implementacion del proceso de enfermeria.

A clinica como pratica arborifica e rizomorfica do trabalho em enfermagem cirurgica

A qualitative and exploratory case study was conducted in a surgery unit of a university hospital. The study aimed to analyze the nurses’ work from the perspective of health care production and clinical practice. The subjects of the study were six nurses. Non-participant observations, documentary research and in-depth interviews were carried out, followed by discursive textual analysis. Nursing work is organized according to two interconnected and interdependent perspectives: a clinical model, which forms the central structure of its practice, and a structure formed by multiple and heterogeneous elements. in this way, the clinical model of health care is organized as a centered structure that enables the fulfillment of biological needs and acts as a basis for connecting disparate knowledge and practices that expand practice through interconnections with the work environment.

CONSTRUÇÃO MORAL DO ESTUDANTE DE GRADUAÇÃO EM ENFERMAGEM COMO FOMENTO DA HUMANIZAÇÃO DO CUIDADO

Objective: to reflect on how the moral construction of the undergraduate nursing student can promote the humanization of care. Method: this is a theoretical reflection based on two different frameworks: Piaget and Kohlberg. Results: the following are mentioned as possible consequences of this reflection: the recognition of the moral dimension of care; the need to consider the culture of the subjects involved in care; overcoming technicality in nursing, also considering the ethical dimension of care and, finally, the provision of instruments to teachers in order to enable them to address human morality as an exercise of citizenship at all academic times. Conclusion: we believe that academic nursing education dedicated to the moral construction can help to prepare nurses who are capable of humanized care
