OBJECTIVE: To estimate the prevalence of no harm incidents and adverse events in a surgical clinic. METHODS: Cross-sectional study conducted with a sample of 750 hospitalizations in the surgical clinic of a hospital in the mid-west region. A descriptive analysis was performed and the prevalence of incidents was calculated. RESULTS: It was demonstrated that 615 hospitalizations were exposed to no harm incidents and 140 to adverse events. Of the 5,672 reported incidents, 218 were characterized as adverse events that caused harm to the patient. No harm was proven for the others; however, they highlighted the need for an adjustment to work processes. CONCLUSION: The prevalence of no harm incidents was estimated at 82%, and 18.7% for adverse events.

Administration of medicines is an ally in the health recovery process, especially in the emergency and casualty sector. Since the nurse is the person with most responsibility in this practice and considering that any failing in this activity could have irreversible consequences for the patient, the study seeks to describe the knowledge of the nurses about this issue and to verify aspects of their training in a search for continuous updating and education in the health service. The study is descriptive and exploratory and of a quantitative approach. It was carried out in casualty and emergency units of the Distrito Sanitario Leste de Goiânia, Goias. 37 nurses participated in the study, answering a self-administered questionnaire. A statistical and descriptive analysis was carried out, taking a confidence interval of 95% and factors with significance p < 0,05. The study shows the training of nurses in terms of medicine administration in casualty and emergency units and highlights gaps that need to be remedied by ongoing training.

This study aims to determine the understanding of nurses working at a sentry hospital of adverse events, to identify the most frequent events and the actions taken by the institution for the safety of care, from the viewpoint of nurses. This is a descriptive, exploratory study conducted during 2009 in a teaching hospital in the Midwest with 50 nurses who answered to a questionnaire. The data were analyzed statistically, featuring the absolute and relative frequencies. Adverse events most frequently cited were hospital infection, medication error, trauma during the procedure, pressure ulcers, falls and burns. Most of them understand the concept of adverse events, though some have pointed out the difficulties. All of them recognized the risks and noted the adoption of measures for monitoring and evaluating the service to improve health care practice.

The aim of this study was to estimate the prevalence and factors associated with the occurrence of incidents related to medication, registered in the medical...The aim of this study was to estimate the prevalence and factors associated with the occurrence of incidents related to medication, registered in the medical records of patients admitted to a Surgical Clinic, in 2010. This is a cross-sectional study, conducted at a university hospital, with a sample of 735 hospitalizations. Was performed the categorization of types of incidents, multivariate analysis of regression logistic and calculated the prevalence. The prevalence of drug-related incidents was estimated at 48.0% and were identified, as factors related to the occurrence of these incidents: length of hospitalization more than four days, prescribed three or more medications per day and realization of surgery intervention. It is expected to have contributed for the professionals and area managers can identify risky situations and rethink their actions.

The studys aim was to analyze the importance assigned by the nursing staff to the recording of vital signs of elderly inpatients, as well as perceived barriers and benefits. Data were collected through interviews held with 13 nurses and the reports were analyzed using content analysis, considering the health belief model proposed by Rosenstock. The categories that emerged from the analysis indicate barriers that interfere in the proper monitoring of vital signs, namely: workload, lack of availability and accessibility of basic equipment such as thermometers, stethoscopes and sphygmomanometers, which compromises the nursing assessment and leads to a greater susceptibility to incidents. Although the facility does not provide conditions to measure vital signs properly, the nursing staff attempts to do what is feasible given their current knowledge and context to achieve the best outcome possible in view of the resources available. Das categorias emergidas, identificamos barreiras que interferem na verificacao adequada dos sinais vitais, tais como a sobrecarga de trabalho, a disponibilidade e acessibilidade a materiais basicos como termometro, estetoscopio e esfigmomanometro, comprometendo a avaliacao de enfermagem e acarretando maior susceptibilidade aos incidentes. Ainda que a instituicao nao forneca condicoes para a afericao adequada dos sinais vitais, desvela-se que a equipe de enfermagem tem tentado fazer o que e viavel diante do conhecimento atual e do contexto inserido, na tentativa de alcancar o melhor resultado possivel, mediante os recursos disponiveis.El estudio tuvo como objetivo analizar la importancia, las barreras y beneficios percibidos por el personal de enfermeria, relacionada con el registro de los parametros de signos vitales en ancianos hospitalizados.
