Pintando novos caminhos: a visita domiciliar em saúde mental como dispositivo de cuidado em enfermagem

Estudo sobre a analise do cotidiano domiciliar do paciente psiquiatrico egresso da internacao hospitalar, por meio da visita domiciliar, dando enfase a compreensao da pratica de Enfermagem Psiquiatrica em domicilio. Abordamos as dificuldades, as relacoes e as possibilidades de cuidado. O estudo e qualitativo com enfoque na etnometodologia. A producao de dados foi por meio de observacao participante e entrevistas abertas. Foi adotado o conceito de cotidiano de Michel de Certeau como referencial teorico. Os temas oriundos das discussoes formaram eixo para o processo reflexivo, tendo como base o entendimento acerca de familia, trabalho, lazer e mito/espiritualidade. Os dados refletem como o cuidado oferecido pela enfermeira na internacao hospitalar tem poucas perspectivas de construcao. A visita domiciliar mostrou a importância da participacao da familia e do sujeito em um processo continuo de cuidado, permitindo a enfermeira construir um cuidado criativo, solidario e sensivel, que possibilite aos sujeitos novos contratos com a vida.

Reflexões sobre o pesquisador nas trilhas da Teoria Fundamentada nos Dados

This descriptive-reflexive study was performed with the objective to present the characteristics of researchers who use the Grounded Theory method, and outline the development of aptitudes for the researcher to become a Grounded Theoretician. The theoretical discussion was based on the frameworks of this methodology and supported by the literature. The article presents the main demands of qualitative studies using Grounded Theory, and important behaviors, attitudes and characteristics developed by the researchers. It is concluded that learning about Grounded Theory involves more than operationalizing a group of procedures and techniques. Enfermagem psiquiátrica: discursando o ideal e praticando o real

Resumen El presente estudio analiza el cuidado de la enfermería en el espacio de la interacción hospitalaria, con énfasis en la comprensión de los presupuestos teóricos incluidos en el proceso del cuidado. En este trabajo presentamos a las enfermeras los presupuestos teóricos que podrían auxiliarlas durante el proceso del cuidado de pacientes con enfermedades mentales. El estudio es de naturaleza cualitativa. El modelo teórico adoptado para guiar las discusiones y el análisis de los datos fue “la intuición empática” de Oliveira. Los temas que surgieron de las discusiones guiaron el proceso analítico y reflexivo. Las enfermeras estuvieron de acuerdo y aceptaron los presupuestos teóricos del modelo “la intuición empática” de Oliveira y consideraron que los mismos son muy valiosos en el proceso del cuidado que ejercen las enfermeras psiquiátricas. Sin embargo, los datos evidenciaron que existe una laguna en la capacidad de actuar efectivamente en una situación real basándose en estos conocimientos teóricos y prácticos. Under this work, we present the nurses theoretical assumptions that may assist the act of taking care of mentally ill patients. This is a qualitative study. It was adopted the theoretic model “suggests the empathy” by Oliveira, to guide the discussions and the analysis of data. The themes that came from the discussions, guided the process analytical and reflective. The nurses agree and accept the assumptions of theoretical model of care “suggests empathy” by Oliveira and they believe these will help them with the care action of the nurses’ psychiatrists. However the data show a gap between the capacity of acting efficiently in a real situation supported in theoretical and practical knowledge. A Universidade extendida: estratégias de ensino e aprendizagem em enfermagem

Es una reflexion sobre la practica de enfermeria que aproxima, teoricamente y de una manera propositiva, las actividades de extension a la ensenanza de pregrado. La ensenanza de enfermeria es enfocada desde la practica de extension universitaria, que tiene que ser basada en el proceso ensenanza-aprendizaje y que representa un plus de refinamiento universitario en la medida en que organiza un movimiento de llevar la universidad para fuera del campus y de traer la ciudad para dentro de la universidad. La extension es una ensenanza viva, en el cual el aprendizado del plan de estudios que estan expuesto a la incertidumbre del saber haciendo. Se reflexiona la practica de la enfermera bajo la oferta de la extension universitaria como una posibilidad de llevar conocimiento a la comunidad y al mismo tiempo aprender con ella.

Um entendimento linear sobre a teoria de Peplau e os Princípios da Reforma Psiquiátrica Brasileira

Trata de uma analise sobre os conceitos contidos na teoria de Hildegard Peplau, datados de 1952 e associa-los aos preceitos da Reforma Psiquiatrica Brasileira considerando a realidade social e historica dessa associacao. Partiu-se da analise sistematica do livro Interpersonal Relations in Nursing e dos conceitos nele existentes. O estudo foi realizado sob o enfoque da metanalise, que ajudou na compreensao dos conceitos estudados. A partir da analise das duas referencias teoricas, compreendemos que os conceitos chaves de Hildegard Peplau ainda hoje sao aplicaveis e capazes de orientar com excelencia para um cuidado de enfermagem psiquiatrica que atenda aos preceitos organizadores da Reforma Psiquiatrica Brasileira.

Liderança em enfermagem psiquiátrica

O processo de lideranca em enfermagem psiquiatrica e uma tematica que deve ser cuidadosamente abordada e amplamente discutida. Contudo, poucas sao as fontes cientificas acerca do assunto. Assim sendo, buscou-se atraves deste ensaio refletir a lideranca e as suas varias formas de abordagem em psiquiatria ao longo dos tempos, sendo possivel certificar sua evolucao. A assistencia de qualidade deve ser o objetivo unico e primordial e a lideranca na enfermagem psquiatrica deve estar baseada no reconhecimento das necessidades desta clientela e nas qualidades de sua equipe. Este muitas vezes constitui um desafio para os profissionais enfermeiros, permitindo um maior crescimento profissional. E e justamente o preparo do profissional que auxiliara no bom andamento do tratamento do paciente psiquiatrico, alcancando um resultado satisfatorio

Um espaço para a enfermagem psiquiátrica e de saúde mental

Conceived in the end of 19TH Century, the Psychiatric Nursing is characterized in Brazil as an assistance hospital model that disciplines the care to the mental patient, until today existing in the country as classic psychiatric model, despite the movement of the Brazilian Psychiatric Reformation that demarcates a new time. At the same time, the Nursing in Mental Health walks in a direction to overcome this paradigm, to “invent” new possibilities of care when giving voice to the citizen, coming back toward to this citizen, their history and subjectivity, and no more and only for the symptoms. This epistemological rupture proposed for all the “psy” areas is configured in the Nursing by the transformation itself of the object of the Psychiatric Nursing, that leaves of being the illness and return itself for the inserted citizen in the social context. Thus, the Mental Health Nursing is characterized by the transition of a hospital practice to discipline the behavior of the “mental patient”, for a practice based in new principles. In the practice of the Psychiatric Nursing, many interventions have in the biomedical model a guide for the advances in the Neuroscience and the psychopharmacs. Concomitantly, it is observed an increasing worry with the social and cultural values, family, leisure, work and house of the “mental patient”. We consider that to make possible this perspective still represents a huge challenge in the daily of the institutional practice and of the society in general, once that contradictory processes are simultaneously started. Therefore, this is a critical period for the profession, but also it is an extremely favorable period for the knowledge construction and analyses on the processes of take care of, teach and research in this area. To share reflections about these questions is the proposal of this thematic number. The objectives established for the organization of this number had been: (a) to extend the literature in the scope of the Psychiatric Nursing and Mental Health; and (b) to offer a space for spreading of the works that has being developed in the Psychiatric Nursing and in Mental Health, in the Brazilian context. The idea of this number was immediately accepted for the Deliberative Board of the Journal, aiming the necessity of to increase its regularity in function of the increasing demand of manuscripts and the recognition of the property and relevancy of the initiative of a number that enclosed this subject. And the initiative was supported finacing by the Carlos Chagas Filho Foundation of Support to the Research of the State of Rio de Janeiro (FAPERJ). In this number of the Journal it is evident the diversity of thoughts and actions around specific interests of the Psychiatric Nursing and of Mental Health. In the first block, the ar ticles analyze the history of Psychiatry and its influence in the models of to take care of the Psychiatric Nursing; the total institutions and the formation process in health politics; and the contribution of the movement of the Brazilian Psychiatric Reformation and the Psychosocial Rehabilitation for the Nursing. A second group of articles approaches the challenge of to work with the new devices of care, specifically in the Centers of Psychosocial Attention (CAPS), and the contribution of the domiciliary visit and the Program of Health of the Family, as technologies of care and spaces of exchange among professional, family and user. One third set of articles analyzes the education and the process of formation of the nurses; and the challenge of to teach from articulated practice experiences with the municipal and state net of health. Still it is debated one another article, the leadership in Psychiatric Nursing for the ethical management and of quality of the care. In synthesis, in this number the authors deal with questions related to the education, to the research and to the care. Therefore, the Anna Nery School Journal of Nursing brings inestimable contributions to the known of the Psychiatric Nursing and Mental Health and why not to say, for the society.
